Disciplinary hearing resulted in dismissal - appeal to be done?

my disciplinary hearing resulted in dismissal last week, i have a right to appeal, what BEST can i write in my appeal. I have worked for the company for the last 3 years and 6 months, i am the best team player, its a call centre, always hit my target, best on the floor, i was given final written warning in dec 2010 for 12 months as i by mistake could not handle the data. Well now its similar act but due to the reason as my mind was totally stuck becoz my son is under dermatology treatment for the last more than 6 months, and for the last two months doctors didn't respond me properly as well and i got stuck into situation at company that again i cudn't handle data, well i am always top in the team, worked in the company since 2008, i am jobless now, my wife does not work, i am the only source of income, i am a father of 2 toddlers, i was performing my best in the company to be promoted one day but i got sacked last week, they gave me a chance of appeal, help me please, i have always been liked by manager, collegues and have many friends at work place. Any suggestions please, can they take me back, please i am stressed so much, can i get this job back, do help me to appeal in the best way, i am taking panadol extras everyday and just been devastated, help me please.
